What Is a Diabetic Foot Ulcer?

A diabetic foot ulcer is an open wound that develops due to a combination of nerve damage, prolonged pressure and poor blood circulation. These ulcers commonly occur on weight-bearing areas such as the sole, heel or toes.

Loss of sensation delays detection of minor injuries, while impaired healing in diabetes allows wounds to persist and progress into chronic, non-healing ulcers.

How Is a Diabetic Foot Ulcer Diagnosed?

Most diabetic foot ulcers are detected late because nerve damage eliminates the pain that would normally prompt attention. Here are two important signs that can help diagnose a diabetic foot ulcer at home:

  • Skin temperature: a warm spot on one part of the foot compared to another can indicate active inflammation or infection; coolness suggests poor circulation. Patients can check this at home by feeling both feet with the back of the hand
  • Skin and nail inspection: peeling, cracking, maceration between toes, thickened nails, and colour changes are early warning signs. Patients can and should do this daily

Once these early warning signs are identified, a detailed clinical evaluation helps confirm the diagnosis and determine the severity of the ulcer.

  • Wound grading system (Meggitt-Wagner classification) stages the diabetic foot ulcer from grade 0 for a superficial wound to 5 for gangrene: the grade determines how aggressively treatment needs to begin
  • Peripheral pulse evaluation: pulses at the ankle and foot are felt by hand; absent or weak pulses signal poor arterial flow and risk of delayed healing
  • Nerve function testing using a monofilament checks if you can feel pressure on your foot: if you can’t, you may have been walking on an injury for days without knowing it
  • Vascular assessment using Doppler ultrasound and ABI measures circulation to the foot: an ischaemic ulcer without adequate blood supply will not heal, regardless of other treatment

Offloading Techniques

  • Offloading prevents repeated trauma and allows wound healing
  • Types of offloading footwear: Non-removable total contact casts (TCC) are the gold standard; other options are removable cast walkers and half shoes for patients who cannot tolerate a TCC
  • Offloading should continue after healing is complete to prevent recurrence
